18-21 Transition Academy

The 18-21 Transition Academy is designed for young adults with intellectual or developmental disabilities (IDD) ages 18 to 21. Our goal is to increase the daily independence of each student by providing them with the necessary skills (communication, socialization, personal health, vocational, etc.), experiences and opportunities within our community.

Who is leading the 18-21 Transition Academy?

Heidi Hamilton, Program and Enrichment Center Director

Heidi was a Special Education teacher and Transition Coordinator at Arrowhead High School in Hartland, Wisconsin, for 29 years. Growing up with a younger brother who has special needs, she developed a passion for helping students/adults with special needs transition into the workforce and adult life.
